The initial pre-pilot is limited to five facility places. This page explains the opportunity; it does not schedule a walkthrough, enroll a facility, or guarantee a start date.
Pilot availability
CoreAxis is accepting pilot interest by email. Availability is reviewed directly by the founder, and placement is not guaranteed.

Use subject CoreAxis Pilot Interest. Include your name, organization, role, facility size if applicable, and main challenge. Eligible facilities are reviewed in received-inquiry order, subject to pilot fit and onboarding capacity. There is no guaranteed placement or start date. Care-partner interest is welcome separately and does not count toward facility places.
Opening your email application does not enroll you or confirm that a message was sent.
